Hospice care is not giving up, hospice care is taking control. At Notre Dame Hospice we help people facing advanced illness live more fully. Our focus is on the individual, making sure you live with advancing illness in your own way. We find out what's important to you and use this as our guide to help you along your journey - making it a time of faith, hope, and love.
In home, In-Patient, Respite and Continuous Home Care. Hospice services are offered in the patient's home, assisted living facility, nursing home, hospital setting or personal care homes.
To us, hospice is about providing the care beyond the cure so individuals can live more fully in comfort, peace and in control. Our mission is to provide quality, compassionate health care and support services in a Christian environment. One which respects the uniqueness of each person, promotes and enhances the quality of life, and preserves dignity in life, illness and death.
Notre Dame Hospice is a ministry of the Archdiocese of New Orleans and is the only Catholic nonprofit hospice on the Mississippi Gulf Coast. We call forth the compassion and kindness of our staff, who serve in this healthcare ministry, to respond to the physical, spiritual and emotional needs of our patients and their families. We believe in the sacredness of life from conception to natural death, and we devote ourselves to supporting patients and families at the end of life's journey on this earth and new life in eternity.
At Notre Dame Hospice we focus on enriching lives. We work with our patients, caregivers and families to promote and enhance the quality of the time that remains. Our hospice care is not focused on the disease but on pain control and symptom management. Our care is also focused on more than your medical needs, we’re here to offer emotional and spiritual support as well. We’ll encourage you and your loved ones to partner with us to allow us to deliver the type of care that speaks to your needs and reflects your wishes. We’ll take the time to learn who you are as an individual and understand what matters to you. Then tailor our care to support your wishes and goals. Hospice services are offered in the patient’s home, assisted living facility, nursing home, hospital setting or personal care homes. There are four levels of hospice care that a patient can receive. They are: Routine Home Care, Respite Care, General In-patient Hospice Care and Continuous Home Care.
Routine Home Care
Notre Dame sees the majority of our patients in a home setting where we provide Routine Home Care. A home setting includes a private home, apartment, assisted living facility or skilled nursing home. Patients receiving routine home care work with a hospice team led by a Registered Nurse Case Manager (RNCM). The RNCM works with the patient and caregiver to design a plan-of-care that is customized to the individual patient’s needs. Plans-of-care typically include visits by an RN, Certified Nursing Assistants (CNAs) social workers, chaplains and trained volunteers. As part of Routine Home Care, Notre Dame Hospice often partners with long-term care facilities to meet the needs of patients with life-limiting illnesses. The Hospice Nurse Case Manager educates the facility’s nursing staff on certain health conditions that are more common among terminally-ill patients, and coordinates skilled and advanced prescription drug pain management care often needed toward the end-of-life. In addition, Hospice CNAs provide personal care to complement CNA services in these facilities.
Respite care is available to hospice patients and their families and provides time-limited breaks for the caregiver. Because Respite is covered by Medicare, MediCAL and private insurance, there is no need to worry that this level of care is beyond your reach. Respite Care is provided in a contracted inpatient facility where our patient will be supervised by an onsite RN and hospice care continues just as it was in the patient’s home with regular visits by Notre Dame Hospice Team Members. Our philosophy of care encompasses the whole family. Respite Care is an example of that and through this level of care, Notre Dame Hospice provides peace of mind and support not only to our patients but to their caregivers as well. In this way, respite care is a win-win situation. The patient gets the level of care they need while primary caregivers receive the well-needed break that they deserve. We will do everything we can to support families through advanced illness and make sure that it is not a journey traveled alone.
If it is determined that you need a level of care that meets Medicare standards for in-patient services Notre Dame Hospice offers advanced care through our beautiful Chateau de Notre Dame and Wynhoven facilities as well as at other facilities throughout the area. Chateau de Notre Dame and Wynhoven were built to evoke a home like atmosphere while providing 24 hour skilled care. Large rooms, areas to gather, and comfortable furnishings add to the setting allowing patients to live like they would in their own home. Patients can also enjoy the company of family and friends without the restrictions of visiting hours. Guest apartments are available for family members to be near their loved ones at all times. We’re here to encircle you and your loved ones with 24 hour care delivered in a warm and loving place.
Continuous Home Care
For those patients who wish to remain at home during brief periods of crisis, Notre Dame Hospice is able to provide Continuous Home Care. During Continuous Care, a member of the Notre Dame Hospice nursing staff is present with the patient and caregiver in the home for a minimum of 8 hours followed by consecutive 8 hour periods for as long as deemed necessary by hospice staff. We understand that these are difficult times for you and your family, but we want you to know that as a primary caregiver, you aren’t expected to be able to provide around the clock care to your loved one and manage a busy family life. We want you to know that feelings of guilt or abandonment are normal, but setting realistic expectations for yourself are a healthy way of dealing with this difficult time. Continuous Home Care can be a great time to spend a few precious hours on yourself or to catch up on other errands. Above all we want you to know that we are here for you and your family.
We invite you to become a part of a special person-to-person network. Our Ambassadors help us to increase the awareness and understanding of hospice services, so that more people may take advantage of this important aspect of end of life care. At Notre Dame Hospice, we believe in the sacredness of life from conception to natural death, and we devote ourselves to supporting patients and families through the dying process at the end of life’s journey on this earth and new life in eternity. We are looking for people who may be interested in learning more about hospice and are willing to help us in our educational endeavors throughout our entire service area. As an Ambassador, you may speak to a friend individually, speak to a group, club or to the members of your parish. We will provide you with the information and materials. We’ll meet with you and other Ambassadors every other month to share information and experiences. Our Ambassadors are the most effective method of communicating to the community about a service which we hope will reach everyone who needs it. By assuming this role, you assist us in imitating the healing ministry of Jesus Christ as we care for terminally ill patients and their families. Your help is needed and is greatly appreciated. We need you as an Ambassador. For us, it is all about Faith, Hope and Love.
We understand that your advancing illness is not only touching your life but your family’s life as well. So we’ll be there to comfort your family and help them through any challenging days ahead. Through our Bereavement Services we provide support for those who have lost a loved one by helping them adjust, begin to heal, and ultimately regain the ability to re-invest in life. Bereavement services are available for 13 months. Beyond 13 months, the Church can help as long as there is a need. Remember at no point in this journey will you walk alone because we’ll be walking right there next to you.
